Interior Tile Choice #1
The greenish tile (color appears darker here), accented by the river rock tile would be in the bathroom. The small tiles in the middle would be used as accept pieces throughout, as well as being the bulk of the desktop at the reception desk. The grouping on the right would be the floor throughout the entry, waiting area, and hallway.
Interior Tile Choice #2
This tile would run throughout the office (except patient rooms which will be carpet), and the bathroom would still have the river rock accent pieces worked in somehow, but with this on the floor.

OK, you know I couldn't just shop for the business, so here are some plans and ideas I have added to my Mental Dream House. I found them all at the world's most awesome little tile shop in Green Bay.

I vote for color. How often will you really want to change the decor anyway?
I'm going with #2, the neutral. I like color too, but think the neutral will be easier to keep clean and consistent. Both are great though. :)
I'd go for the green combo. It's still neutral enough to adapt with your decor and it's a little less...patio tile? Is that what I'm thinking? Haha.
I vote for the top choice. The color makes it feel very welcoming and soothing... that "homey" feel. I think, too, that the colors aren't so bold that you can't mix up the decor over the years if you felt you needed to.
Regardless, all great choices!
Okay.........I hate to sound redundant but I like the color. If you're anything like me then if you plan to add things "later" you'll end up always planning and only adding when it's convenient. And if your exactly like me, you'll have half of it finished for like ten years and find yourself making excuses for it everytime someone walks in. Then you'll say "Stupid stupid boy. Why can't you just accomplish anything?" and then you'll find yourself at the bottom of a bottle of pills and rum and.......
oh nevermind. I like the color. Let's just keep it at that. ;)
